The area of autonomous systems is expanding at an incredible rate, with Human Machine teaming systems appearing in a number of very different sectors. The Human Factors team in Thales have been involved in a number of incident investigations, assessing cases where this teaming has broken down. The data presented comes from a UAV programme and highlights some of the fundamental challenges that we are facing while trying to adopt this new technology and ways of working.
