Now you have had your place confirmed with us, there's a few things you need to make sure you have done before you start.

Please ensure you have completed your online occupational health questionnaire at www.healthclear.co.uk.
You’ll be asked for a username and password which have been sent to you by our Student and Academic Support Service team. You will be asked to change your password when you access the website for the first time and you can also edit your personal details if they have changed recently. If you do not have a username and password, please contact our Student and Academic Support Service on 01904 321321.
It is important all your details are correct, in particular your email address and phone numbers, as we may use them to contact you. The Occupational Health Department may contact you for further information or ask you to attend an appointment. Please note it is imperative you attend all Occupational Health appointments when invited. Failure to do so may result in you being unable to commence the programme.
We’ve also supplied you with a form which needs to be completed by your GP, so please ensure that this is filled in and returned to us.
You should also contact your GP to obtain a record, stamped and signed by the practice, of all vaccinations you have previously been given. It is essential that you have had two doses of MMR vaccine and your GP or Practice Nurse should be able to update these if necessary. We also need to know if you have had chickenpox. Your vaccination information should then be returned on the Occupational Health Vaccination and Blood Test Information Record (previously sent to you with your offer letter) in the prepaid envelope provided by the Student and Academic Support Service. If you haven’t already done so, you’ll need to complete an online questionnaire for the Disclosure and Barring Service (DBS). To enable you to access the Disclosure and Barring Service application online, you will be required to answer a security question. We've set this up prior to your application and will use your mother’s maiden name as the security question. If you haven’t already done so, please can you e-mail your mother’s maiden name to karen.downes@york.ac.uk clearly marking your email ‘private and confidential’. Once this information has been received, you will receive an e-mail from crb@york.disclosures.co.uk, with the link and details of how to start your Disclosure and Barring Service application.
Please could you also send proof of your address, for example a bank statement, credit card statement or utility bill, but these must original copies and be recent, i.e. less than three months old. Please send to Karen Downes, Student and Academic Support Service, Ground Floor, Seebohm Rowntree Building, University of York, Heslington, York, YO10 5DD.
Once you’ve done this, you’ll receive a certificate from the DBS which you need to keep in a safe place for the duration of your course. There’s no need to send the certificate to us.
Please remember to send a copy of your academic qualifications, if you have not already provided them, to Student and Academic Support Service. This is particularly important if you have been waiting for GCSE's, Functional Skills or Degree certificates.
For the postgradute nursing programme it is a condition of re-enrolment that you agree to maintain professional indemnity insurance. When working on placement for a healthcare organisation, arranged through the university, you will generally be covered by the organisation’s indemnity arrangements. However, in some of the smaller placement providers and any elective experience further cover may be required. This cover can be obtained either through membership of a professional union such as the Royal College of Nursing (RCN)/Unison or from a personal broker. Please note exclusions may apply.
In order to try and find accommodation most suitable for you, it is best to contact the accommodation office direct by email (accommodation@york.ac.uk) or phone (01904 322165) so that they can understand what your accommodation preferences and priorities are before offering you a room. However, as your course is starting part way through the year, we cannot guarantee there will be rooms available on campus.
