The Carer QALY Trap in valuing survival gains in patients with informal carers

Abstract: The ‘classic’ QALY Trap describes the impossibility of assigning equal value to life extensions for persons in full health and those with some chronic shortfall in quality-of-life whilst also valuing a cure for the chronic condition. Here we discuss an analogous “Carer QALY Trap”, whereby it is impossible to assign equal value to life extensions for patients with and without informal carers whilst also valuing reductions in the caring burden. At the extreme, this Carer QALY Trap can imply that life extensions for patients with positive utility but a high caring burden have negative societal value within the QALY framework.
