Issues in conducting a comprehensive economic evaluation as part of a randomised controlled trial

Abstract: There is growing awareness that in order to facilitate treatment decisions in health care, randomised control trial (RCT) findings may be more informative when used to update existing evidence on all treatments of interest, rather than being considered in isolation. The NIHR-funded VenUS IV study (Venous leg Ulcer Study IV: A randomised controlled trial of compression hosiery versus compression bandaging in the treatment of venous leg ulcers) is committed to presenting its cost effectiveness findings in relation to all high compression treatments for venous leg ulcers. However, in practice there is limited guidance on how such analyses should be conducted and presented. 

Existing literature suggests that, to date, decision models conducted as part of an RCT incorporate only some trial data where as VenUS IV was designed to fully inform a decision model. This seminar will consider how we might achieve this. We will discuss the alternative approaches for synthesising VenUS IV data with external evidence on outcomes and costs for all relevant interventions, highlighting the strengths and weaknesses of alternative approaches. We will also discuss on-going methodological challenges around structuring a mixed treatment comparison to estimate treatment effectiveness for all relevant interventions.
