Before the use of any hazardous substance you must complete a COSHH assessment. In addition, before ordering you must determine the Hazard Severity Level (see poster A3 or A2) - this MUST be included on your order form (see HSE (IS) 15).

The Department COSHH form will help you complete a COSHH assessment. To use the form complete page 3 first, then pages 1 and 2.
